Holiday Food Basket Program Request Form

The Community Action Partnership of San Bernardino County Food Bank strives to alleviate hunger, malnutrition, and poverty throughout San Bernardino County by mobilizing the necessary resources to create positive health outcomes for those suffering from hunger and food insecurity. The holiday time often exemplifies food insecurity experienced by families and individuals residing in San Bernardino County. 

 

Each year,  the CAPSBC Food Bank receives a substantial influx of requests to serve the community with food support during the holidays and we work to fulfill as many requests as we are able. It is not possible to fulfill every request due to the high volume received. Approved requested will be limited to a maximum number of turkeys and/or turkey breasts which may or may not match the number requested. All holiday requests must be submitted using the electronic request form on this page and only one request submission is allowed per organization and per holiday.

​2020 has certainly been a challenging year and food insecurity has tripled in California as a result of COVID-19. Due to the unique conditions in San Bernardino County, this year’s Food Bank Holiday Basket Program requests must detail your plan to adhere to public health requirements in order to be COVID-19 compliant.

Thanksgiving Holiday Request Forms have been closed on October 23, 2020 - 5:00 PM.

Christmas Holiday Request Forms will be accepted until November 13, 2020 - 5:00 p.m.

The following information outlines CAPSBC Food Bank’s holiday distribution program requirements:

Please review all Holiday Basket Program information prior to submitting a request.

Requirements

  • The organization must be a 501(c)(3)

  • The organization must exclusively serve low-income individuals/families residing in San Bernardino County

  • CAPSBC allocated resources must be provided to the community at no cost

  • The organization must adhere to the Holiday program agreement and utilize CAPSBC marketing materials

  • The organization is to provide required reports within 5 days of the event Late request submissions will not be considered

Conditions

  • Priority is given to existing CAPSBC Food Bank partner agencies

  • Requests from organizations approved in 2019 that did not submit required reports will not be considered

  • Organizations who are non-compliant with the submission of required 2019 paperwork will not be considered

Approval of Holiday Food Bank Requests 

CAPSBC Food Bank maintains sole discretion over the approval of all requests. Priority is based upon the Food Bank’s determination and suitability, and organizations must meet the detailed requirements.

 

Request Approval Process

After the request period closes, all organizations submitting a request during the respective request period will receive a follow-up email with the determination of the request. If the request was approved, then the next steps will be detailed in the message.

Please note that the submission of an application and meeting program requirements does guarantee approval of the request.
